Motivation in ADHD Has Been Studied Only Through Reward
A theoretical paper on what four decades of ADHD reward experiments left unexamined.
What the paper argues
For most of the past forty years, motivation in ADHD has been treated as a question about reinforcement. The influential models describe a dopamine system that responds atypically to reward and punishment, and the experiments built on them ask a narrow version of the question: does a token or a small sum of money improve performance on this task. The answer is generally yes, and somewhat more so than in other children.
That framing has begun to loosen. Writing in the Journal of Attention Disorders in 2022, Sarah Morsink and colleagues at KU Leuven, with co-authors at the universities of Amsterdam, Ghent and Radboud Nijmegen, argue that the field has measured one end of motivation and inferred the rest.
Their paper reports no new data. It is a theoretical review, an argument about how a field should frame its questions, closing with proposals for future studies rather than results. It follows no systematic search protocol and is not a meta-analysis; its central claim is a position rather than a finding.
Motivation, in the wider psychological literature, refers to internal motives: the needs, thoughts and feelings that stand between something happening to a person and what that person then does. A parent asks a child to start their homework, and whether the child starts depends on what the request meets inside them. ADHD research, the authors write, has recorded the request and the homework and left the middle of that sequence unmeasured.
The framework they propose to fill it is Self-Determination Theory, which describes motivation as a continuum running from none at all, through behaviour performed under external pressure, to activity undertaken out of genuine interest. Movement along that continuum is said to depend on three psychological needs: autonomy, the sense of being the origin of one’s own actions; competence, the sense of being effective; and relatedness, connection to other people.
Morsink and colleagues leave the reward findings themselves standing. A 2005 review by Luman and colleagues found that reinforcement improved performance in children with ADHD and in controls, with the improvement more prominent in the ADHD group, and a 2016 meta-analysis by Ma and colleagues found that reinforcement could bring inhibitory control in children and adolescents with ADHD up to the baseline level of controls. What Morsink and colleagues question is what such studies reveal about motivation, given that the tasks are arithmetic drills, continuous performance tests and repetitive motor sequences, selected for being long, dull and hard for these children especially. The authors state plainly that low intrinsic motivation across all of life is unlikely in ADHD, and that high interest in some activities alongside little in others is the more probable case, as it is for everyone.
What it means for you and your child
A child who sustains four hours on a model or a game and cannot begin twenty minutes of French is easily read as choosing, because the capacity is so visibly present elsewhere. The paper offers a reason to hold that reading loosely. The question of what actually engages a child with ADHD has rarely been put to them directly; it has been put in the form of whether a reward raises a score on a task nobody would choose.
Where the field has asked, the answers have some texture. In a 2019 questionnaire study, Morsink and colleagues found that children and adolescents with ADHD reported being as motivated as their peers by tasks that were rewarded, competitive, socially evaluated or predictable, and less motivated than peers by tasks that were cognitively demanding, effortful to focus on, collaborative or graded. In a separate qualitative study, Sibley and Yeguez recorded young people with ADHD describing real absorption in high-interest subject matter and vocational work, and in settings that granted them latitude and drew on what they were already good at.
Autonomy, competence and relatedness give a parent three questions that a reward chart does not raise. Whether the child had any say in how the work was approached. Whether they expect to be capable of it. Whether anyone whose opinion matters to them is involved. Rogers and Tannock reported in 2018 that among primary school children in a non-clinical sample, higher levels of ADHD symptoms were associated with lower reported satisfaction of all three needs in the classroom, though the design was self-report at a single point in time and cannot say which condition preceded the other.
None of this recommends abandoning reward. Reinforcement demonstrably improves performance, and two studies that looked for the classic cost of rewards, a decline in willingness to do something for its own sake once payment stops, did not find it in children with ADHD. The interventions built on Self-Determination Theory have not been tested in an ADHD population at all, a gap the authors name as their own.
What they are asking their field to do, in the end, is to measure enjoyment, choice and interest rather than only accuracy under incentive. You have watched for years which conditions absorb your child entirely and which cannot hold them for ten minutes. That is the observation the research has not yet troubled to collect.
