Nineteen Studies, Six Dimensions, One Uneven Profile

A 2025 systematic review gathered nineteen empirical studies on twice-exceptional students and found a consistent signature: sharp internal contrasts within the same child, across cognition, learning and social life.

What the research found

A twice-exceptional child is gifted and also has a disability. The two things sit together in one person: strong reasoning alongside, say, attention difficulties, autism, or a specific learning disorder. A 2025 systematic review by Luana Rizzo, Stefania Pinnelli and Alexander Minnaert set out to describe what these children actually look like when you gather the evidence in one place.

The authors searched four academic databases (Web of Science, SCOPUS, Education Source and ERIC) for studies published between January 2013 and January 2025. That window is deliberate: it starts with the DSM-5, the diagnostic manual that reshaped how disabilities are classified. The search returned 906 records. After screening titles, abstracts and full texts against strict criteria, 19 empirical studies remained. To count as gifted, participants had to score 120 or above on a full-scale IQ or an equivalent composite index. The children studied were up to 19 years old, and the most common profiles were giftedness paired with ADHD, with autism spectrum disorder, or with a specific learning disability.

The review read each study through a multidimensional lens, sorting findings into six areas: cognitive, metacognitive, academic, social, emotional and physical functioning. The headline result is not a single number but a pattern. Across the cognitive studies, twice-exceptional children showed large internal discrepancies. Their scores scattered widely across different subtests rather than clustering. In one striking comparison, gifted children with ADHD showed gaps between working memory and processing speed that were nearly twice as large as those seen in children who had ADHD alone. The giftedness did not smooth over the difficulty, and the difficulty did not erase the giftedness. Both persisted, side by side.

The second finding concerns social and emotional life. Here the picture was consistently harder. Studies reported bullying, exclusion, a weakened sense of belonging and isolation. Advanced ability in one area did little to protect these children socially, and in some cases the mismatch between their strengths and struggles appeared to compound the strain.

It is worth being precise about what this review can and cannot show. A systematic review gathers and organises existing studies. It does not run a new experiment, and it cannot establish cause. It can tell us that certain features recur across the literature; it cannot tell us why, or in what proportion of all twice-exceptional children they appear. The authors are candid about the limits. Only 10 of the 19 studies included a comparison group, which weakens confidence in the contrasts. Sample sizes ranged from single case studies up to 680 participants, so the studies are not equally weighty. The methods varied widely (7 were qualitative, 9 quantitative, 3 mixed), making direct comparison difficult. And because inclusion depended on IQ-based identification, studies that defined giftedness in other ways were left out. The much-discussed idea that one exceptionality can mask the other, the authors note, remains genuinely hard to capture in data.

What it means for you and your child

If your child seems to hold two contradictory truths at once, quick to grasp big ideas yet stuck on tasks that should be easy, this review offers a measure of reassurance. That combination is not a contradiction to be explained away. It is the documented signature of twice-exceptionality. The uneven profile is the thing itself, not a sign that someone has misread your child.

The practical implication follows from the numbers. Because the internal gaps can be large, a single overall score, whether an IQ figure or a school grade, will often mislead. A child whose reasoning is excellent but whose processing speed lags may look merely average on a summary measure, with both the gift and the difficulty averaged into invisibility. When you review an assessment, look past the headline number to the subtest breakdown. Ask directly where the peaks and troughs sit. The spread between them is where the useful information lives, and it is exactly what this body of research keeps finding.

The social findings deserve equal attention. The evidence points repeatedly to exclusion and a fragile sense of belonging, and these do not resolve on their own because a child is clever. If your child is thriving intellectually but withdrawn or unhappy among peers, treat that as a real and separate need rather than a phase that ability will outgrow. Support for the strength and support for the struggle are two jobs, not one.

Two cautions keep this honest. First, this is a review of 19 studies, several of them small and without comparison groups. It describes patterns in a young field; it does not deliver settled facts, and it certainly does not describe any individual child. Second, the studies included only children identified as gifted by IQ of 120 or above. Children whose gifts show up in ways a test does not capture may be twice-exceptional too, and simply absent from this evidence. Use the findings as a well-organised map of what researchers have observed so far, not as a diagnosis. The most reliable guide to your particular child remains a careful, individual assessment, read with attention to the contrasts within it.
