The Rating Scale and the Lab Disagreed
A study of 149 Gulf-region children found broad executive-function difficulties in autism on parent-rated measures, yet the child’s planning looked almost normal on a computerised task. Both pictures are real. The gap between them is the point.
What the research found
Executive function is a plain idea wrapped in a technical name. It is the set of mental skills that let a child hold a goal in mind, plan the steps toward it, resist the first impulse, and adjust when things change. This study asked a simple question about children with autism spectrum disorder (ASD): are these skills impaired, and does the answer depend on how you measure them?
The researchers assessed 119 children with ASD and 30 typically developing children, aged 6 to 12, recruited across three Gulf states: Bahrain, Saudi Arabia, and the United Arab Emirates. Crucially, they measured executive function two different ways. The first was a parent questionnaire, the BRIEF-2, which asks how the child manages in everyday life. The second was a set of computerised tasks, the CANTAB battery, run under controlled laboratory conditions. Same children, same skills, two very different lenses.
On the parent questionnaire, the picture was stark. Across nearly every domain, children with ASD scored far higher on difficulty than their peers, with large statistical effects. The single most impaired area was planning and organisation: 74.8% of the children with ASD scored above the clinical cutoff, the threshold clinicians use to flag a real-world problem. Emotional control followed at 62.2%, self-monitoring at 60.0%, and both cognitive flexibility (“shift”) and working memory at 59.7%. The broad summary score, the Global Executive Composite, was elevated in 74.8% of the children. Not every domain was severe, though. Organisation of physical materials was flagged in only 19.3%, and task-monitoring in 36.1%.
Then came the laboratory tasks, and the pattern shifted. On the computer, children with ASD did show real difficulties in cognitive flexibility, working memory, and inhibition, each a statistically significant gap. But on the planning task, the Stockings of Cambridge, there was no significant difference between the two groups at all. The very skill that parents rated as most impaired, in three of four children, looked essentially preserved when the child sat at a screen and solved a structured puzzle.
That is the study’s central finding, and the authors are careful about it. They describe a “divergence between the informant measures and the performance-based findings” that “could reflect a poor overlap between the two approaches.” Their overall conclusion is measured: children with ASD experience genuine executive-function difficulties “in both everyday and laboratory settings, albeit not in the same areas.” They also stress heterogeneity, noting that not every child with autism struggles in any given domain.
What this means for you and your child
The most useful thing this study offers a parent is permission to hold two truths at once. Your child may genuinely struggle to plan and organise the messy, open-ended demands of a real day, and also perform well on a neat, self-contained puzzle. Neither result is wrong. They are measuring different things that share a name.
Why the gap? A laboratory planning task hands the child a clear goal, a fixed set of moves, and no distractions. Everyday planning offers none of that. It means starting homework without a prompt, keeping a multi-step morning routine going while a sibling talks, and adjusting when the plan meets reality. The lab measures whether the machinery works. The questionnaire measures whether it runs under load. For many children with autism, the machinery is intact but the load is where things fall apart. That is a hopeful reframing: the goal is often to reduce the load, not to rebuild a missing skill.
Practically, this points toward scaffolding the environment rather than drilling an abstract capacity. Visual schedules, checklists, consistent routines, and clear one-step-at-a-time prompts take the planning burden off the child’s shoulders in exactly the situations where it collapses. A child who can solve a screen puzzle but cannot start their homework does not need more puzzles. They need the real-world task broken down and made visible.
It is also worth registering what a single study like this can and cannot establish. It shows a correlation and a pattern, not a cause. It describes a snapshot in time, so it says nothing about how these children change as they grow; the authors themselves call for longitudinal work. Its 30-child comparison group was small, and it deliberately included only children with milder, higher-functioning autism, so the findings may not extend to the wider autism population. And the two measures disagreeing does not tell us which one is “right.” They are simply different windows.
The honest takeaway is not a headline number but a stance. Be sceptical of any single profile that claims to sum up your child’s executive function in one figure. A parent report and a clinician’s test can point in different directions and both be honest. If you are seeking an assessment, ask for more than one kind of evidence, and ask the person interpreting it to explain the gaps rather than average them away. The gap is not noise. Often, it is exactly where the useful information lives.
