Time Perception in ADHD Is Really Several Different Skills

The phrase covers at least four separate tasks, and a Prague review wants the diagnostic manual to name them.

What the research found

A person is given a task to complete and then asked how long it took. In one arrangement they are told before they start that the question is coming, so the passing minutes can be attended to while the work goes on. In the other they hear of it only once the task is over, and the duration has to be reconstructed from whatever the memory of the task happens to contain. The first is called prospective time estimation and the second retrospective, and a person can be competent at one while failing at the other.

Those two procedures dominate the ADHD timing literature, according to a 2021 review in Medical Science Monitor by Simon Weissenberger, Radek Ptacek and colleagues at the First Medical Faculty of Charles University in Prague. Their argument is that temporal difficulty is a central symptom of adult ADHD rather than a secondary consequence of inattention, and that it should enter the next revision of the American diagnostic manual.

Two further tasks appear under the same heading. Time reproduction presents an event of a certain length and asks for a span of the same length in return, with no numbers involved at either end. Time discrimination asks only which of two intervals was the longer.

The review reports difficulty across most of these. Children with ADHD reproduce a previously presented duration poorly, and the authors state that this was not caused by intellectual disability or by weak working memory. Retrospective estimates diverge from those of comparison groups, including children carrying other psychiatric diagnoses. Prospective tasks are described as difficult and estimation as inaccurate, in a direction the authors summarise as time seeming to move faster than it does.

Discrimination is the exception, with no behavioural result cited, only reduced connectivity between frontal regions and the cerebellum while such a task ran. One condition reversed the pattern outright. When computer-based timing tasks included emotional material, among it pictures of facial expressions, children with ADHD outperformed children without the diagnosis, in work by Nazari and colleagues published in 2018.

The review reports no statistics. It is a narrative review with no stated search protocol, and it gives no sample sizes and no measure of how large any of these differences were. Almost all of the evidence it assembles was collected in children, which the authors acknowledge while arguing for a conclusion about adults, and their own word for that conclusion is belief. The proposal that time perception is focal to ADHD has not been tested. What the review does demonstrate is that one phrase has been standing in for several different measurements, and that those measurements come apart.

What it means for you and your child

The distinctions earn their keep by naming things a parent has already seen. A child who cannot say whether the shower took five minutes or twenty five is at the retrospective end. A child who agrees, accurately, that the walk to school takes a quarter of an hour and still leaves at the wrong moment is contending with something separate, the forward estimate that must be made before anything has happened.

Reproduction has fewer household counterparts. It strips out language and planning and asks the child only to match a span they have just experienced, and children with ADHD were reported to perform it poorly.

Time monitoring is a fifth term in the same family, and the most visible of the set. Children without ADHD check the clock more often as a deadline approaches, while children with ADHD, on the review’s account of a quicker internal clock, do not adjust in that way. A child who glances up once at the start of an hour and not again is showing the monitoring difference, which is the one an adult in the room can observe without any equipment.

Precision of this kind is worth having in a school meeting, where poor time management is heard as a statement about character and effort. A sentence describing what the child cannot do before beginning a piece of work points somewhere other than motivation, and it is the kind of description a specialist assessment can follow up.

Any parent quoting this paper should quote it as an argument rather than a result. It cannot say how many children are affected, or whether the adults it is written about show the profile of the children it draws on. Its recommendation to the manual’s authors is a recommendation, and the field has not yet acted on it.

The lateness that arrives with no apparent cause, and the homework that was going to take twenty minutes and took the evening, each have a name in this literature and a task built to measure them. Nobody has yet counted how far apart the two groups sit. The review supplies a vocabulary for something you have been watching for years.
